Before we send this out to lots of people, could you guys and girls please fill this out and also leave any feedback / suggestions / typos / etc. as a reply here?

I think it's great.  Has all the information you need to receive.  Couple of points

 

"We are setting up a new community run not-for-profit food co-op to provide fresh fruit and veg boxes at an affordable price."  - Do you want to remove 'at an affordable price' since that is one of your choices for joining the scheme.

 

Would you be willing to become a member of the co-op and volunteer on a regular basis in order to get cheaper prices? - you might want to split this in to 2 questions (a yes/no question and a field to capture the number of hours).  it will be easier to analyse.

One thing you might want to add is how they would want to be contacted and what payment method.

 

for contacting - either email, text, website.

 

for payment method - willing to pay by direct debit? cash?

 

And maybe a question about holidays?  would they like to either not pay for when they go on holiday (so changing their payment) or would they like to pay a fixed amount and have a set number of holiday weeks?

have made some changes, see what you think

 

have taken in what you've said and am still trying to keep it short, i think we can ask more detailed questions later further down the line?
